ROMS & the New MVA Treatment Plan/PAF Forms:

·  Quickly identify pre/coexisting conditions, MVA impairments, ADL/work limitations, rehabilitation barriers

The New *OCF 18 Treatment & PAF Forms: New Requirements and Tools to Address Them

Under Bill 198, as of October 1, 2003, the revised OCF-18 (Treatment Plan) and the OCF-22 (Assessment Application) will be required for all claims with accident dates  after November 1, 1996, while the following forms will be required for all claims after October 1, 2003: OCF-23 (PAF Confirmation); OCF-24 (PAF Discharge/Extension Request).
